Secondly, Aerococcus urinae also tends to be resistant to the antibiotics Trimethoprim and Ciprofloxacin, both commonly used to treat UTIs. Patients often only start to feel better if given active antibiotics such as Amoxicillin or Nitrofurantoin. Thirdly, Aerococcus urinae tends only to cause UTIs in the
Aerococcus urinae is a newcomer in clinical and microbiological practice, causing urinary tract infections, bacteraemia/septicaemia and/or endocarditis. This study presents for the first time an evaluation of the activity of a representative panel of antibiotics against a large number of A. urinae isolates.

In complicated UTI, ampicillin is probably a safe treatment option, whereas nitrofurantoin is likely effective in uncomplicated UTI. Comparative 16S rRNA gene sequencing demonstrated that the organism represents a new subline within the genus Aerococcus. The unknown bacterium was readily distinguished from the three currently recognized Aerococcus species, Aerococcus christensenii, Aerococcus urinae and Aerococcus viridans, by biochemical tests and electrophoretic analysis of whole-cell proteins.

A. urinae is a gram-positive coccus that grows in pairs and clusters, produces alpha-hemolysis on blood agar, and is negative for catalase and pyrrolidonyl aminopeptidase. Nitrofurantoin was the most common treatment and had 71-76% effectiveness in treating one type of aerococcus (A.urinae) and 42-50% in another type (A. sanguinicola).

Pivmecillinam was successful in bladder UTI caused by A.urinae. Ciprofloxacin was effectively used in patients with pyelonephritis (kidney infection). Aerococcus urinae is a significant cause of UTI (3%)and infective endocarditis (>90% if seen in blood culture) Treatment of Aerococcal UTI should ideally be with Amoxicillin OR Nitrofurantoin Treatment of Aerococcal infective endocarditis should be with Amoxicillin PLUS Gentamicin Aerococcus is Aerococcus urinaeis an uncommon urinary tract pathogen that causes infections predominantly in elderly persons with local or general predisposing conditions. During a one-year study, the clinical features ofAerococcus urinaeurinary tract infections (≥105cfu/ml) were investigated in two large medical microbiology laboratories in the Netherlands.
Den första arten, A. viridans, beskrevs 1953 och kopplingar gjordes mellan den och en allvarlig sjukdom hos hummer. Det skulle dröja åtskilliga decennier innan nästa aerokockart, A. urinae, upptäcktes. Aerococcus urinae is a rarely reported pathogen, possibly due to difficulties in the identification of the organism. While the concentration of thymidine in human urine and serum is typically low (25), it may vary depending on a pa-tient’s dietary folate intake.

Aerococcus urinae is a Gram-positive alpha-hemolytic bacterium first isolated and identified from urine from patients with UTI (8). Since its discovery, A. urinae has been isolated from a wide variety of patients, both male and female, ranging from children to the elderly (9).

Aerococcus urinae is a rare organism isolated from urine cultures. We present a case of an 80 year-old male with bladder cancer and multimorbidity who developed A. urinae infection. A. urinae may cause simple and complicated UTIs, bacteremia, and endocarditis in older adults with multimorbidity, chronic urinary retention, or indwelling catheters. Aerococcus urinae can, in older males with underlying urinary tract conditions, cause invasive infections such as urosepsis or infective endocarditis. The prognosis of invasive aerococcal infections appears to be relatively favourable despite the old age of patients and their many comorbidities. Aerococcus urinae is a rarely reported pathogen, possibly due to difficulties in the identification of the organism.
